Only for water. Water has a density of exactly 1.000 g/mL, so 1 mL = 1,000 mg. For other substances the conversion depends on density — 1 mL of honey = 1,420 mg, while 1 mL of alcohol = only 789 mg.
Because milliliters measure volume (space) and milligrams measure mass (weight). These are different physical properties that cannot be directly compared. Density — the mass per unit volume, measured in g/mL — is the essential bridge between the two units.
For water-based solutions (density ≈ 1 g/mL), 5 mL ≈ 5,000 mg of total liquid. However, for medication dosing always refer to the concentration on the label (e.g., "250 mg/5 mL") rather than converting the total liquid mass — the active ingredient is only a fraction of the liquid.
